OFFICIALS’ WRITTEN REPORTS

All injuries must be reported by email to: lino@ontarioballhockeyfederation or faxed to (289) 316-1881. By filling out the form below and clicking the Submit button, your form will be delivered directly to the OBHF Head Office.

Officials are to inform the League Operator of all serious personal injuries to spectators, officials, players, or team officials. Officials are to complete and submit the for below within 24 hours of the incident occurence.
